Implementation notes

Vincent 1996 itself has no abstract in PubMed and was not read. Gaps in the printed table resolved here: bilirubin "6.0–11.9" then "> 12.0" — 12.0 exactly scores 4; creatinine "3.5–4.9" then "> 5.0" — 5.0 exactly scores 4; dopamine exactly 5 is unassigned in the table (choose the closer description). P/F < 200 without respiratory support scores 2 (the table requires respiratory support for 3 and 4). Renal points = the higher of creatinine and urine-output points. Baseline assumed 0 unless known pre-existing dysfunction, and SOFA ≥ 2 ≈ 10% mortality, from Sepsis-3 Box 3 / text.
