Implementation notes

Intervals per Marshall 1995 (SI units; conventional units converted). BUG FIXED 2026-10-11: converted values landing in the rounding gaps between bands (e.g. creatinine 1.13125 mg/dL = 100.004 µmol/L) threw an error; exclusive lower bounds are now explicit.
