Implementation notes
The S12 equations are the paper's logistic regression approximations of the full competing-risk models (R² ≥ 0.99 at 10 years, ≥ 0.97 at 30 years); they are the published implementation. One mismatch: the paper prints 30-year HF 19% for the non-smoker; the S12 equation gives 18.5% (published value may come from the full model). UACR 40, HbA1c 7.5, SDI decile 8) for women and men — these are the only primary-source male values; the paper gives no male example. Ranges are the supplement's "allowable range" (age 30–79, TC 130–320, HDL 20–100, SBP 90–200, BMI 18.5 to < 40, eGFR 15–150, UACR 0.1–25,000, HbA1c 3–15, SDI 1–10). The supplement applies the 30-year equations across ages 30–79; whether the AHA online calculator restricts 30-year estimates to younger ages was not checked. Model choice when add-ons are entered (single-predictor model for one, combined model with missing indicators for two or more) follows the table structure; the paper does not state which set its online calculator uses. BMI is optional: without it the heart-failure estimates are withheld; total CVD and ASCVD equations have no BMI term. CHD and stroke equations are parsed but not shown.
