Implementation notes
Thresholds per IMWG 2014. Rajkumar's own Table 1 in Am J Hematol 2016 (PMC4715763) agrees on CRAB thresholds, ≥ 60% plasma cells, FLC ratio ≥ 100 with involved FLC ≥ 100 mg/L, > 1 MRI focal lesion ≥ 5 mm, and the smoldering/MGUS definitions.
