Days of fever

Infant ≤ 6 months with fever ≥ 7 days and no other explanation Bilateral bulbar conjunctival injection without exudate Oral changes: red cracked lips, strawberry tongue, or oropharyngeal erythema Polymorphous rash Extremity changes: red/swollen hands and feet (acute) or periungual peeling (subacute) Cervical lymphadenopathy ≥ 1.5 cm, usually unilateral CRP ≥ 3.0 mg/dL and/or ESR ≥ 40 mm/h Supplemental: anemia for age Supplemental: platelets ≥ 450,000 after day 7 of fever Supplemental: albumin ≤ 3.0 g/dL Supplemental: elevated ALT Supplemental: WBC ≥ 15,000/mm³ Supplemental: urine ≥ 10 WBC/hpf Positive echo: LAD or RCA Z ≥ 2.5, aneurysm, or ≥ 3 suggestive features
