Implementation notes

The footnote also says "4 mm is subtracted from the actual nodule size", but the printed transformation already centres size at 4 mm ((4/10)^−0.5 = 1.58113883); size enters only through the transformation. Risk = logistic transform of the linear predictor (the paper describes logistic-regression models; the formula itself is not printed).
