Implementation notes
Gestational-age groups follow the tables exactly: phototherapy without risk factors has separate 35, 36, 37, 38, 39 and ≥ 40-week series; the other three tables group ≥ 38 weeks. Age uses completed hours (hour 0 is not tabulated; input from 1 h). Series that end before 359 h are constant afterwards per their footnotes; ages ≥ 360 h are outside the tables and rejected. Escalation threshold = exchange − 2 mg/dL (KAS 17, verbatim). Risk-factor list from guideline Table 2 / table captions (GA < 38 weeks is handled by the GA choice). KAS wording paraphrased from the guideline text. Supersedes the 2004 AAP / Bhutani calculator (bili_aap2004).
