UpToDate alternatives in 2026: an honest landscape

Most UpToDate alternatives are other reference libraries, a few are AI answer engines, and one category is not a reference at all. Here is what each does, with prices where they are published.

See also our free clinical calculators, each with its primary references.

People look for UpToDate alternatives for three different reasons: price, a preference for a different style of content, or a need UpToDate was not built for. Which alternative fits depends on which of those it is. This is the landscape as of October 2026, with every fact taken from the vendor's own site.

UpToDate, for reference

UpToDate's topic reviews are written and reviewed by over 7,600 expert clinicians, editors and reviewers, with recommendations graded using GRADE, and users can earn CME while they read.[1] Its generative AI feature, UpToDate Expert AI, is included in all US subscriptions according to Wolters Kluwer, and UpToDate Connect offers an API to digital health companies.[2] Individual subscriptions are sold through its online store; the store was offline for maintenance when we checked, so we have not quoted a price. DynaMed (EBSCO)

DynaMed is a point-of-care reference covering more than 35 specialties, updated multiple times daily, with CME and MOC tracking. An individual subscription is $399 a year, or $475 a year with its Dyna AI feature (individual Dyna AI is currently US-only), with a 14-day free trial.[3]

Choose it if you want a full reference library at a published price, with frequent updates.

ClinicalKey AI (Elsevier)

ClinicalKey AI combines Elsevier's evidence-based content with conversational search powered by generative AI, and each response includes citations linked to published evidence. Elsevier lists SMART on FHIR access in the EHR and describes it as API-enabled.[4] Its subscription page lists $349 a year after a 90-day free period, or $39 a month; another Elsevier landing page shows $499 a year, so confirm the price you are offered.[5]

Choose it if you want AI answers with citations into full-text journals and books from a large publisher.

BMJ Best Practice

BMJ Best Practice lists a US physician subscription at $435 a year (or $39 a month), with lower prices for trainees, other health professionals and students, built-in CME tools, and a comorbidities manager that it describes as unique among CDS tools.[6]

Choose it if your patients commonly have several conditions at once and you want management guidance that accounts for them.

OpenEvidence

OpenEvidence is an AI answer engine rather than an authored library. It is free for verified US healthcare professionals and has signed content agreements with NEJM, JAMA, Nature, NCCN, Cochrane and others.[7] Its security page says covered entities may transmit PHI under its standard business associate agreement.[8]

Choose it if you want fast, cited answers and cost is a deciding factor. Free is hard to beat.

Pogosh: not a reference, a different job

Pogosh is not an UpToDate alternative in the reading sense, and it would be misleading to list it as one without saying so. It has no topic library and does not award CME. It reads a clinical note and returns structured, coded next steps: labs with LOINC codes, imaging with RadLex codes, specialist routing adapted to setting and patient, escalation criteria, a strict emergency check, relevant calculators offered (never auto-run), and a drafted note. It is delivered as a REST API with a HIPAA business associate agreement on AWS and as an OpenEMR module, at a published $149 to $299 per month per location.

Choose it if the problem is not "what does the evidence say" but "what should be ordered next for this patient, in a form the chart can use".

How to choose

If the reason you are leaving UpToDate is price, DynaMed, ClinicalKey AI and BMJ Best Practice all publish individual prices, and OpenEvidence is free for US clinicians. If it is format, decide between authored topic reviews and AI-generated answers with citations. If what you actually need is the plan in the chart rather than a better thing to read, that is a different category, and it is the one Pogosh is in. Whatever you choose, Pogosh's output, like any tool's, is advisory; a clinician reviews it and can override it.
